Provides optimal nutritional care and diabetes self-management education to outpatient pediatrics endocrinology practice by assessing nutritional needs and developing nutritional care plans using the nutrition care process.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Bachelor’s degree in Nutrition or related field.
  1. National Registry with Commission on Dietetic Registration (CDR).
  2. Certified Diabetes Educator (CDE).
  3. WV Board of Licensed Dietitians (WV BOLD).

EXPERIENCE:

  1. Two (2) years’ experience as a Registered Dietitian (RD) in a hospital, long-term care facility or outpatient setting.
  2. Six (6) months experience providing diabetes education, working with children and adolescents.

1. One (1) year of experience providing diabetes education working with children and adolescents.   1. Interviews, assesses and develops plan for the institutional needs of clinic patients, in collaboration with attending physicians.
  2. Counsels individuals, families, etc. on insulin administration, insulin pump settings and diabetes management through individual sessions and group classes.
  3. Manages insulin titration per physician orders.
  4. Maintains effective written and verbal communications with physicians' offices and school nurses.
  5. Documents education and assessment of outpatients in the electronic medical record.
  6. Maintains nutrition clinic records and participates in QA activities.
  7. Utilizes and develops resources and educational materials for patient education.
  8. Develops and maintains Diabetes Education program clinical quality initiatives.
  9. Facilitates community education and outreach specific to nutrition and diabetes management.
  10. Liaison to pharmaceutical pump and sensor representatives.
  11. Educates families on insulin pumps, continuous glucose monitors, and other advances in diabetes.
  12. Develops protocols for ongoing education assessments.
  13. Triages messages including providing sick day management for diabetes (keeping them out of the ED).
  14. Advocates for patients for supplies and school accommodations.

SKILLS AND ABILITIES:

  1. Effective verbal and written communication skills.
  2. Professional knowledge and skill as a diabetes and health educator.
  3. In-depth knowledge and understanding of diabetes, up-to-date nutritional disorders, dietary plans, related issues and patient care concerns.
  4. Ability to investigate, analyze information and to draw conclusions.
  5. Attention to detail and action oriented.
